Documents required for registration of the passport of Ukraine:

• Ukrainian passport

• ID

• Men under 25 years of military card or a certificate from the recruitment office (if not served)

• If you previously had a passport you must either surrender or leave. To leave you need to bring a copy of the first page.

For inclusion in the passport data on children:

• A certificate of birth

• If a child older than 2 years old then you need to bring 4 matte color photos 3×4

• The passport and the passport of the parent of a child who will be pasted

To make child ticket:

• 4 matte color photos 3×4

• A certificate of birth

• Ukrainian passport of a parent. If the parents are registered in different places that need help from ЖЕКа Form number 3

• Notarized permission of both parents in the design of children’s ticket
